Required hard-copy documentation for new student registration can be brought to the school, Monday-Friday, 8:00-3:00.  Please call ahead to notify the school office, 850-936-6020.

WHAT TO BRING:

• Birth Certificate

• Current Physical (dated one year prior to start date, or later)

• Proof of Residence such as a current utility bill, lease agreement, or sales contract.

• Florida Immunization Record (updated on Florida form)

• Custody or Guardianship Papers (if applicable)
